Mid-Atlantic Association of Community Health Centers invites you to come join our staff and guest lecturers for our 27th Annual Meeting and Conference. Our theme this year, From Purpose to Practice: Innovation to Strengthen Healthcare, represents the crossroads our industry is facing. Earlier this year our government demonstrated its faith in the community health center movement through unprecedented financial support. Our health system will undergo fundamental changes over the next five years, and the role of community health centers in the new system that emerges, could be great. It is up to all of us to ensure that this happens.
From Purpose to Practice: Innovation to Strengthen Healthcare will focus on key areas of emerging changes that are occurring in our health system today that directly impact your patients and your bottom line. Our learning sessions have been developed with the guidance of health center staff to ensure that we are bringing together the right people to answer the right questions. MACHC’s goal for our annual meeting and conference is that community health centers have the tools and information they need to be the leaders in the new health system paradigm.

Session Descriptions
While our choice of venue is a lean back and relax locale, this conference will be a psh-up your sleeves get ready to work engaged learning sessions! Each session is a minimum of 2 ½ hours long – this is to allow for meaningful discussion, engaged learning and in-depth information sharing.
This conference will offer a total of 5 different sessions; three concurrent sessions in the morning and two in the afternoon.
You can either choose from a morning and afternoon session, for a total of two session. The descriptions of our target-learning audience will help you choose which session is most pertinent to your roles.
MORNING CONCURRENT SESSIONS (Choose one of the following)
Risk Management at All Levels- Communicating a Culture of Accountability
It’s everyone’s responsibility to be aware of the potential risks that organizations can be exposed to during daily routine operations and clinical functions. A routine clinical scenario will be presented and dissected to reveal the prospective risks and liability exposure to the community health center at the patient, organization and leadership levels. Understand the relationship between risk management and financial, employee skill and competency, work ethic, organizational trust and customer satisfaction can impact the organization.

Meeting Meaningful Use Panel Discussion – What it means for your organization and how to get there.
Understanding a 850 page federal rule is daunting enough – implementing it to take advantage of the financial incentives it will offer, however, could make it all worthwhile to your organization. This session will be a panel discussion of leaders in the development of the Meaningful Use Rule to discuss with you the elements of Meaningful Use that will have the greatest impact to your organization. The session will also include the introduction of a HIT/EHR Gap Analysis tool to help ensure your organization meets the Meaningful Use deadlines.

Billing and Coding for Health Centers Optimization
Does your organization chronically “under-code” visit levels? Is provider productivity a constant topic of conversation? Do various rules and guidelines around Evaluation and Management (E&M) coding seem baffling? This program will review appropriate code type and level selection (e.g., 3 of 3 rule versus 2 of 3 rule versus time), will cover documentation of the key components (SOAP), and dispel myths around clinicians not wanting to code the highest levels of established patient E&M coding. Attendees will also receive an intensive ICD overview and enhanced understanding of encounter rate within the FQHC environment.

Transitioning and Positioning – Making Patient Centered Medical Home Model a Reality
This session will focus on understanding the Patient Centered Medical Home Model through the lens of a comparative analysis. How does your health center’s current services and delivery model compare to National Committee for Quality Assurance’s requirements for the various Levels of PCMH readiness? Furthermore, what can your health center do to move your CHC into PCMH readiness? In addition, we will have a discussion from the State and Federal officials on the newly launched Patient Centered Medical Home initiative, the corresponding reimbursement models and how to get involved.

Speakers Biographies
Keynote Speaker
Mr. John Cafazza - Director of the Central Mid-Atlantic Division for the Bureau of Primary Health Care (BPHC) Health Resources and Services Administration, U.S. Department of Health and Human Services
John Cafazza has worked in the field of community-based health care delivery since 1984, having acquired a depth of experience in the areas of partnership building, advocacy and primary care infrastructure development and expansion. In Mr. Cafazza’s current role as Director of the Central Mid-Atlantic Divisvion, he leads the Division’s efforts to support the Consolidated Health Center Program grantees in Regions 3, 5, and 6, working with Health Center grantees, Primary Care Associations, and Health Center Controlled Networks. Now nearly a $2.0 billion Federal investment in over 1,100 health care organizations nationwide, the Health Center Program provides primary care services to more than 18 million of the nation’s most vulnerable people, the poor and underserved residents of rural and frontier areas, isolated areas, and urban inner cities. Health Centers are a major component of the Nation’s health care safety net with over 7,500 delivery sites.
Mr. Cafazza’s initial exposure to primary care came as an administrator of a rural community health center in southeastern Virginia. There Mr. Cafazza learned firsthand the people, the organizations, and the systems that create the “medical home” necessary for those typically unable to access care in the mainstream health care system. From that position, Mr. Cafazza moved to a statewide focus, spending 14 years with the Virginia Primary Care Association (VPCA), initially as its Marketing Director in 1987 and subsequently appointed as its Executive Director in 1992. In the position of Executive Director of VPCA, he learned the importance and necessity of the statewide infrastructure needed to support the local primary care delivery systems. Mr. Cafazza also focused on health center development, primary care delivery support systems and services, workforce development, advocacy and partnership development.
Having joined the BPHC in 2001, Mr. Cafazza has served as the Director of the Office of State and National Partnerships; the Acting Director of the Division of State and Community Assistance; the Acting Director of the Division of Health Center Development; and the BPHC Director of the Division of Health Center Management. Other previous employment opportunities have given Mr. Cafazza experience in the public and private sectors, providing him with knowledge of state and local government operations, as well as experience in the non-profit business world.
Mr. Cafazza is a graduate of Virginia Polytechnic Institute and State University (Virginia Tech).
Mr. James (Jim) Hursey - Chief Operating Officer, Baltimore Medical System
Jim Hursey serves as the Chief Operating Officer of Baltimore Medical System (BMS). Jim works directly for Jay Wolvovsky, President and CEO of BMS. As COO, Jim is responsible for the operations of BMS’ Federally Qualified Health Centers. Jim is also responsible for BMS’s facilities and other key programs, including the organizations pharmacy operations and school-based health services.
Jim joined BMS in 2009 with a strong background in health system leadership. Key areas of focus included the following:
Prior to joining BMS, Jim served as the Vice President of Business Development, Planning and Marketing with St. Joseph Medical Center (SJMC), a 370-bed community medical center. Jim’s responsibilities included strategic planning, physician relations, marketing and public relations, physician enterprise services, legislative and community affairs. In this capacity, Jim also served on the board of key SJMC joint ventures, including its imaging venture, ambulatory surgery center and an outreach venture with St. Agnes. Jim was also the executive sponsor of SJMC’s Planning Committee and chairman of the Political Action Committee.
Prior to joining SJMC in 2006, Jim served as Senior Vice President of the Greater Baltimore Medical Center (GBMC), an integrated health system that includes a 300-bed academic community medical center, an inpatient and outpatient hospice, and multiple physician and ancillary joint ventures. Over a 16 year career at GBMC, Jim’s principal responsibilities included business development, strategic planning, decision support, physician relations, managed care, marketing and public relations.
Jim possesses a strong set of skills essential to his role at BMS. Over his career Jim has leveraged these skills across the health delivery system, including hospitals, joint ventures and the physician enterprise arena. He holds an MBA from the University of Baltimore, and an undergraduate degree in Business Administration/Economics from McDaniel College in Westminster, Maryland.
Ms. Cheryl B. Jones - Director of Outreach, CRISP
Cheryl Jones joins CRISP with more than eight years experience in the clinical and operational aspects of the healthcare industry. Prior to joining CRISP, Cheryl was employed by Erickson Retirement Communities. Her roles included serving as a Six Sigma internal consultant and a Senior Business Analyst, reporting to the executive team. Cheryl was also responsible for the administrative management of the 170-bed skilled nursing and assisted living facility at one of Erickson’s largest campuses.
Earlier in her career, Cheryl spent nearly two years as a healthcare consultant with BearingPoint (now Deloitte). She completed projects at several hospitals across the United States, focusing on revenue cycle optimization and other financial issues. Serving as a BearingPoint consultant, Cheryl was instrumental in assisting the CDC with the Hurricane Katrina After Action Report.
Cheryl holds a B.A. in Psychology from Spelman College and an M.B.A. and M.H.A. from the University of Florida. She has a background in human resources and has received her P.H.R. certification.
Mr. Raymond T. Jorgensen - CEO, Priority Management Group
Ray Jorgensen is President & CEO of Priority Management Group, Inc. (PMG). He is responsible for oversight of consulting operations as well as coding, reimbursement, and payer related issues for the out-sourced billing component of PMG’s services (800,000+ annual encounters). Ray has personally trained thousands from over 35 states on coding, billing, and reimbursement in addition to authoring two books and dozens of articles. Ray’s health care experience and education is unique in that he was schooled by the payers. Having worked for Blue Cross and Blue Shield as well as United HealthCare Corporation, primarily in professional relations and contracting, Ray has an understanding and perspective on the payer’s objectives and process unlike other medical business consultants groomed from the provider side.
Mr. Daniel Wilt - Program Director, Regional Extension Center, CRISP
Mr. Wilt is the Program Director for the Healthcare IT Regional Extension Center for CRISP. He brings more than 15 years of experience with implementing Electronic Health Records (EHR) and the healthcare background to lead the effort.
Prior to CRISP he was at CareFirst BlueCross BlueShield as an Enterprise Architect where he worked on the Primary Care Medical Home Pay for Performance and the payer based Personal Health Record initiative by helping to define the architecture and technology solutions. He was also a contributor to the company’s Healthcare IT policy and analysis along with the point for Health Information Exchanges in the Mid Atlantic region for the company.
Previously he served as Vice President of Information Technology leading the Erickson Health technology initiatives for 5 years. He oversaw the implementation of EHR’s in the Ambulatory, Skilled Nursing Facilities, Assisted Living Facilities, Rehab, and Home Health lines of business. His team developed a personal health record system to provide secure web based access to the resident’s medical information. He was the organizations HIPAA Security Officer and implemented administrative, technical, and physical controls to achieve compliance.
He is an author of many articles focused on EHR’s in extended care including the article co-authored in Journal of Health Information Management titled “Crossing Barriers” and a contributing author to the book “Understanding and Designing Your Active Directory Infrastructure.” Mr. Wilt holds a B.S. in Business Administration from Drexel University’s LeBow College of Business and an Executive M.B.A. from Temple University’s Fox School of Business.

The 2010 Board Excellence & Service Award will be presented to one distinguished board member or an entire board, who by their achievements in governing a community health center; have contributed significantly to enhancing the overall health and well-being of their community and society at large.
Healthcare HERO Award
The Healthcare HERO Award will be presented to an individual or team, whose work has made a significant contribution in the delivery of primary care to vulnerable populations, thereby improving quality, accessibility and/or outcomes of care. The Healthcare Hero Award is presented to those whose work exemplifies employee excellence: promoting human dignity through actions characterized by great personal initiative, selflessness, compassion and imagination. It is also meant to showcase career achievements, and identify individuals who can serve as an inspiration for others.
The blinded entries will be judged and selected by the Annual Meeting Planning Committee.
